That sounds familiar. It might be either "Standing at the Gates of Hell" by
Carol Clarke or "End of the Line" by Yoshi. In either case if you liked that
one, Phoen Dusk wrote a short, dark story called "Torn Apart".

Hi,

I'm searching for a fanfiction which I have read about 9 month ago, but
I only remember the end.
It's a story where buffy kills herself in the bathtub with a blade by
cutting her wrist. At the same time the rest of the scoobies sit on the
dinning room table and know what buffy is doing.

I hope someone know it.
