According to all the information I have heard, though I can't say that I've
seen this special case on any official site, or in any of the eps, by
myself, but then, I don't usually look.

The reason for "Elisabeth" being the name many writers refer to, is because
Joyce has supposed to want to give that name to her daugher. But the nurse
misunderstood the name as "Buffy" during Joyce giving birth (you can expect
her to not give the most clearest answers while being in that amount of
pain) and wrote that name onto the birth certificate.

That's why every 'official' institution has her by the name "Buffy", because
that IS her legal name. "Elisabeth" is merely what Joyce had wanted to give
her, but in the past 20 years, you'd assume they'd just gotten used to
Buffy. =)
