Point of fact: The phrase was not created by
the media. The phrase was hanging on a banner behind Bush when he
delivered a speech on an aircraft carrier in Iraq. Here is a picture
of Bush in front of said banner and a brief article about it.

Point of fact: Bush never said "mission accomplished". The phrase was
created by the media in an attempt to portray that speech as something
it wasn't. What he did was declare "the end of major combat", which is
something completely different.
