Forrest was the one who died--twice. Skewered by Adam, he was rebuilt
into a similar human/demon Frankenstein, but was destroyed by Riley in
"Primeval" (episode 4.21). Check out some screencaps at BuffyWorld or
Romance-on-btvs, if you don't remember.

As far as we know, Graham Miller is fighting demons with Riley around
the world. Or maybe not.
